  Pre Arranged Absence Form

   The Pre-Arranged Absence form must be completed by student’s teachers, signed by parent, letter attached explaining reason for absence, and turned in to the student’s grade level office to be approved by an administrator 3 days before the requested absence.(Ex:  if the student is requesting to be absent on Friday, he/she will need to turn the form in no later than Tuesday at 3:15pm)
